Our mission is to provide the best health and performance for the student athletes of Oregon Tech through the continuous pursuit of new knowledge and application of best practices in the field of sports medicine. Our goal as clinicians is to create an innovative and collaborative culture to optimize the care for each student athlete. To do this, we will utilize an evidence-based practice multidisciplinary approach. This will enhance injury prevention, wellness education, and create effective management of athletic injuries and illnesses.

Athletic Training is an allied medical profession. Athletic trainers help prevent, recognize, and evaluate athletic injuries and provide immediate care, rehabilitation, and reconditioning services to athletes. Support for student athletes is provided by a team of two associate athletic trainers, an orthopedic surgeon, and a family practice physician.


FOR INCOMING FRESHMEN / TRANSFER STUDENT-ATHLETES
  1. Join and Complete Sportsware (Instructions for Sportsware) *If you had Sportsware previously, you MUST use a different email!!!! The school ID to create an account: #Owls
    1. Fill in all pages of information: general, emergency contact, insurance, medical alerts, medical history, etc. 
    2. Complete the PHQ9 form for mental health screening.
    3. Insurance page: attach front and back pictures of your primary insurance card. 
    4. Attachments page: 
      1. Upload copies of your HIPAA/Consent Form, NAIA Consent Form and Athletic Insurance Questionnaire: please save them as first initial of first name then last name and HIAD 2023. For example, jdoe HIAD 2023
      2. Upload copy of completed physical form: please save them as first initial of first name then last name and PPE 2023 (For example, jdoe PPE 2023). NOTE – only an MD, DO or PA can sign off on physicals (we will not accept anything else – chiropractor, nurse, etc).
      3. Attach any other supplemental documentation here such as return post-surgery, etc.

  3. NAIA Consent Form: please print and sign
  4. Athletic Insurance Questionnaire: please print and sign
  5. Medical History/Physical Examination: Physicals will be completed for all freshmen and/or first-year student-athletes. You will need to fill the medical history part out and take it to your appointment for the physician to review.  The last two pages titled "Physical Examination" are the pages that the physicians will fill out. Please make sure that they initial and sign all the appropriate places. If you are missing signatures, or the appropriate boxes are not checked- the form is INVALID.  NOTE – only an MD or a DO can sign off on physicals (we will not accept anything else – PA, chiropractor, nurse, etc).

FOR RETURNING STUDENT-ATHLETES
  1. Update Sportsware:  (Instructions for Sportsware)
    1. Update School Year
    2. Update medical history AND MAKE SURE TO CHANGE THE DATE (in the upper left corner)
    3. Update any changes to address, emergency contact, or insurance information
    4. Upload new pictures of your insurance cards: front and back. 
    5. Complete the PHQ9 form for mental health screening

  5. Health Review: please print and sign. If an athlete has had surgery, a surgical note / clearance note and a return to play protocol must be provided.
